Bariatric Protocol for Prospective Bariatric Patients

 

1. Initial Consultation with Dr. Quilici and the bariatric coordinator. 

You initial qualification requirements will be verified [height, weight, BMI, obesity history and previous weight loss program].  Bring a history of your dieting efforts including dates and weight loss if possible as well as the nutrition/lifestyle form from our website. Educational materials about the surgical procedures and diets are given at this time. If you do not have computer access, please let us know two weeks before your appointment so we can send you the form.  Go home and read your materials. You will be scheduled to undergo a psycho-social evaluation . You will be asked to join our support group network.

 

2. The Bariatric Coordinator will schedule a psycho-social evaluation. Once the psycho-social evaluation is completed, call Dr. Quilici’s office for your second appointment.  When your psycho-social report is received we will apply for pre-authorization for surgeryYou should attend a support group before the second visit.

 

3. Second appointment.  If your surgery is already pre-authorized, we will set-up a surgery date at that time.  Bring your signed informed consent form at that time.  You should sign up for a nutrition class and pre-op support group session during this visit. Call your primary care physician to set-up an appointment for a history and physical 5 days prior to your surgery date.  We will need your doctor’s fax number so we can forward the pre-operative instructions.

 

4. You are now ready for your surgery. Please read the Pre-operative Instructions.
